Certified copy of live Birth Certificate Form

Marriage certificate of parent(s)

Provision for return mail of the documentation

Form BI-529 (To be completed by the South African citizen parent).

Important:

(The forms must be completed in black ink. No corrections must be made on the form. Documentation completed incorrectly will be returned.) Completion of Form BI-529

If the applicant is a major (i.e. 21 years of age and older) the following applies:

Section A: Particulars of the applicant

Section B: Particulars of the applicant's father

Section C: Particulars of the applicant's mother

If the applicant is a minor (i.e. under 21 years of age) the following applies:

Section A: Particulars of the applicant's parent (who is a S.A. citizen)

Section B: Particulars of the applicant's grandfather (father of parent who completed Section A)

Section C: Particulars of the applicant's grandmother (mother of parent who completed Section A)
