- BI-529 Determination of Citizenship form must be completed in respect of child
- BI-529 Determination of Citizenship must be completed in respect of the South African parent

- Four (4) recent immigration size photos of applicant (35mm width x 45 mm height)
- Certified copy of the South African Identity Documents of the Informant and dual citizenship exemption documents if they acquired foreign citizenship as well
- Certified copies of parent Identity Documents, Marriage Certificate and Birth Certificate
- Certified copy of Unabridged Birth Certificate for births abroad
