Re-registration of birth certificate

Processing time: six months

  • BI-24 Notice of Birth (persons under one year) Please request forms from the Consulate General in Toronto or South African High Commission in Ottawa by sending in a self-addressed stamped envelope (size A4 30cm x 24 cm) . The envelope should be able to fit an unfolded page.
  • BI-24/1 Notice of Birth form (2 forms needed); one form must be completed with the current surname and the other with the father’s surname (persons one year and older but under 15 years). Please request forms from the Consulate General in Toronto or South African High Commission in Ottawa by sending in a self-addressed stamped envelope (size A4 30cm x 24 cm) . The envelope should be able to fit an unfolded page.
  • BI-529 Determination of Citizenship form must be completed in respect of child
  • BI-529 Determination of Citizenship must be completed in respect of the South African parent
  • Certified copy of the applicant’s Unabridged/Abridged Birth Certificate
  • Certified copy of parent’s Marriage Certificate
  • Certified coy of parents South African Identity Documents/Foreign Identity Documents
  • Fee – none
  • A self- addressed express post envelope is required with each application.

All forms to be completed in BLACK ink.
